No-Show Policy

Important Notice

At Evansville Primary Care, our patients are extremely important to us. We strive to schedule existing and new patients in a timely manner and to meet your healthcare needs. Therefore, it is vitally important to attend every scheduled appointment.

While we make every effort to remind patients of their appointments, it is the patient's responsibility to keep track of appointments. Our practice requires a 24-hour notice if you are unable to keep your scheduled appointment.

Starting April 15, 2024, Evansville Primary Care will implement a revised

No-Show Policy.

1) For any missed appointment, or if you cancel under 24 hours prior to your scheduled appointment time, a $20.00 fee will post to your account.

2) After 3 missed appointments, you will receive a notification letter by mail, a MyChart notification, and a phone call.

3) After 5 missed appointments, you will be notified of termination as a patient of our practice.
